Employment Type : Full-time (35 hours per week)

Role Summary : The Case Manager will be expected to manage a client caseload while providing them with a comprehensive range of onsite, person-centered, strength-based services. The Case Manager may also play a pivotal role in client counseling, intake, case management, and/or providing essential aftercare services. The primary objective of the Case Manager will be to empower assigned clients to lead fulfilling lives within the local community.

Primary Job Responsibilities/Duties :

The job duties of the Case Manager include, but are not necessarily limited to, the following:

• Promptly conducting clients' initial intake assessments and addressing any critical needs.
• Completing bio-psycho-social assessments of clients within the first 30 days of their initial intake to the facility.
• Providing assessments of client needs toward recommending treatment modalities when appropriate.
• Identify clients who might benefit from information pertaining to alcohol and/or substance use or abuse.
• Utilizing assessment tools (potentially found in CARES and elsewhere) to identify available resources and support services and make timely referrals to those which seem most appropriate based on your understanding of clients' individual circumstances and goals.
• Orienting clients to their assigned transitional housing facility and the larger New York City shelter program while informing and reminding them of the rules, policies, procedures, and schedules currently in effect.
• Providing clients with information and guidance on how to access or apply for available resources, such as public assistance, and alcohol or substance use or abuse treatment services.
• Developing Independent Living Plans ("ILPs"), in coordination with other social services providers, which are designed to deliver comprehensive support services to clients.
• Monitoring and tracking how clients progress in achieving their stated goals.
• Empowering clients to actively participate in their own planning and goal setting.
• Scheduling and conducting meetings with clients (on a regular one-on-one basis and as otherwise needed) to touch base, assess, and address whether there are any updates to their personal circumstances and needs.
• Referring clients for and encouraging them to take advantage of employment and job or skills development opportunities if appropriate.
• Connecting clients with suitable resources to help them achieve their goals.
• Meeting clients to address challenges, successes, and strengthen their capacity for long-term self-sufficiency.
• Maintaining thorough records, including client files, and documenting conversations, warnings, and progress reports, all while preserving appropriate levels of confidentiality.
• Reporting critical incidents immediately to the Site Director and/or their designated onsite representative.
• Attempting to determine client-readiness for employment and housing, potentially in collaboration with Housing Specialists and other Social Services staff.
• Treating all clients, visitors, and employees with kindness, respect, and dignity.
• Serving as an advocate for clients and also connecting them with outside advocacy groups when necessary.
• Providing clients with periodic case management and counseling, extending one (1) year beyond their final exit from the facility.
• Establishing and/or nurturing connections and partnerships with other programs and units, both within and outside the organization, to facilitate referrals and support services.
• Coordinating and/or escorting clients to housing viewings, and DHS or HRA appointments as required.
• Facilitating suitable long-term housing placements for clients as quickly as possible.
• Submitting complete case notes within 24 hours of completing any client meeting.
• Offering support as a substitute for other staff when needed.
• Performing general clerical duties.

Physical Requirements :

• Ability to walk, stand, kneel, crouch, and bend over.
• Ability to lift, carry, reach, and handle objects weighing up to 25 pounds.
• Ability to sit at a desk and work on a computer for prolonged periods.
• Ability to travel to different locations within New York City, as needed.

Work Environment / Schedule Requirements :

• May at times be required to work evenings, weekends, and/or holidays based on organizational needs, deadlines, or events.

Qualifications :

• Bachelor's degree in social work, psychology, or a related field, or an equivalent combination of education and/or experience.
• Must be able to read, speak, write, and understand English for administrative purposes.
• Strong written and verbal communication skills.
• Bilingualism is preferred.
• Exceptional interpersonal skills, with the ability to balance compassion and firmness.
• Always maintain confidentiality.
• Proficiency in navigating community resources.
• Strong problem-solving, decision-making, conflict resolution, and active listening skills.
• Capacity to remain calm and composed in crisis situations.
• Demonstrated ability to collaborate effectively with diverse groups.
• Proven capability to manage multiple tasks efficiently under pressure.
• Outstanding organizational skills, attention to detail, and efficiency.
• Possessing maturity, integrity, and sound judgment.
• Must pass a drug screening test to be appointed.
• This position may be subject to a series of checks before and/or after appointment.
