Erie Family Health Centers, a top workplace in Chicago and suburbs, is looking for a valuable addition to our Community Health Registered Nurse team! Erie Family Health Centers is seeking a full-time Community Health RN to provide essential clinical support to patient care activities utilizing nursing education, assessment, and procedural skills.

Compensation is based on each candidate’s experience, skills and education within the range identified for the role. Candidates who meet the minimum requirements of the role will start at entry in the range. Any additional skills, experience and education will be reflected in the compensation offered.

Main Duties & Responsibilities

\n

\n
• Collaborates with providers in the delivery of patient care.
\n
• Performs triage assessment process of walk-in patients and telephone calls.
\n
• Delivers health education related to medication, diet, treatment of diagnosis, health promotion, prevention and exit health teaching.
\n
• Administers immunizations, applicable contraception refills, and medications as ordered by provider.
\n
• Performs other duties as assigned.
\n
• The work schedule for this role is aligned with Erie's hours of operations.

Qualifications

Education

\n

\n
• Current Illinois RN licensure required.
\n
• Current BLS for Healthcare Provider CPR certification required.
\n
• One to two years’ experience in community health nursing or ambulatory care preferred.
\n

Skills and Knowledge

\n

\n
• Bilingual English/Spanish is required .
\n
