Registered Nurse Case Management

Coordinates management of care and ensures optimum utilization of resources, service delivery, and compliance with external review agencies. Provides ongoing support and expertise through comprehensive assessment, care planning, plan implementation, and overall evaluation of individual patient needs. Enhances the quality of patient management and satisfaction to promote continuity of care and cost-effectiveness through integrated case management functions. Manages utilization review and discharge planning effectively. Serves as a patient advocate through resource utilization, discharge planning, and addressing the holistic needs of the patient. Ensures patient preferences are respected throughout the care process.

Minimum 5 years of recent RN Case Management experience License: California RN license required Certifications: BLS certification through the American Heart Association required Must-Have: - Experience in a Long-Term Acute Care or Short-Term Acute Care Hospital setting - Familiarity with MCG and discharge planning across acute care settings - Strong interdisciplinary team collaboration
