Nurse Navigator - Indian River Family Practice

Location and Schedule Location: 3860 S Straits Hwy, Indian River, MI Schedule: Monday – Friday – Day Shift – Can work 8 or 10‑hour shifts A Day in the Life Act as a liaison between patients, professional staff, and physicians by providing care management and coordination for adult and pediatric patients with complex conditions, social needs and educational needs. Serve in an expanded health care role to collaborate with primary care providers, specialists, and patient/families to ensure the delivery of quality and cost‑effective health care services. Assess plans, implement, coordinate, monitor and evaluate all options and services with the goal of optimizing the patient's health status and decreasing hospital and emergency room utilization. Support quality improvement initiatives and adherence to safety standards.
