Part Time Direct Support Professional

Position Summary Direct Support Professional - PRN at Horizon Health and Wellness . This role involves flexible, as‑needed shifts during the holiday season, with additional opportunities thereafter. Responsibilities Provide essential care and support services to individuals with intellectual and developmental disabilities. Assist with community integration and inclusion, offering guidance to individuals and their support networks. Create meaningful opportunities for participation in community activities. Ensure the health, safety, and overall well‑being of residents through proper supervision. Pick up shifts as needed during the holiday season and beyond. Qualifications High School diploma or GED. Must be 21 years of age or older. Valid driver’s license with an acceptable driving record and reliable transportation. Ability to prepare required paperwork and document activities within contractually required timelines. Typing proficiency and working knowledge of computers and electronic clinical record keeping. Ability to successfully complete certification and trainings for First Aid, CPR, Article 9, Medication, Prevention/Support, and Direct Care Worker (training provided if not possessed). Ability to operate independently with minimal oversight. Demonstrated respectful communication skills with participants and coworkers. Must obtain and maintain a fingerprint clearance card and pass a background check along with Central Registry Clearance. Willingness to work nights, weekends, holidays, and a varied weekly schedule. Preferred Qualifications Bilingual English/Spanish or English/Sign Language. Prior experience in habilitation or rehabilitation. Two years of experience in a person‑centered community integration program assisting persons with developmental and/or psychiatric disabilities. Very strong interpersonal skills.
