New Franklin Center For Rehabilitation and Nursing is seeking an experienced Wound Care Nurse Manager to join our Nursing Department team.

About the Role We are looking for a dedicated and knowledgeable RN to lead wound care management across our facility. The ideal candidate will bring strong clinical expertise, a passion for patient education, and the ability to collaborate across departments to ensure the highest standards of skin and wound care.

Required Qualifications

• Education: RN required; BSN preferred

• Experience: Minimum 2 years in medical/surgical or rehabilitation nursing

• Tech Skills: Comfortable with Electronic Medical Records (EMR)

• License: Valid RN license

• Certifications (preferred): WCC (Wound Care Certified), CWCN (Certified Wound Care Nurse), CWOCN (Certified Wound, Ostomy, and Continence Nurse), CWS (Certified Wound Specialist), or equivalent

• Candidates with extensive wound care experience in skilled nursing/long-term care settings also encouraged to apply

• Strong verbal and written communication skills in English. Multilingual is a plus!

Key Responsibilities

• Perform wound assessments, reassessments, and management per policy; conduct weekly wound rounds

• Apply thorough knowledge of wound staging systems and current wound management theory

• Develop skin care programs to prevent skin breakdown

• Create and implement policies for draining wounds, fistulae/tubes, ostomy care, and incontinence management

• Develop and revise individualized care plans based on patient response

• Provide patient and family education on wound management

• Deliver staff in-services on wound prevention/care, ostomy care, and incontinence

• Consult with interdisciplinary healthcare team members

• Maintain accurate, timely EMR documentation per regulatory standards

• Serve as liaison between physicians, patients, and product manufacturers

• Maintain facility-wide active wound tracking log for quality improvement purposes

• Coordinate care with consulting wound care physician specialists

• Prepare QAPI reports and spreadsheets tracking wound trends

Physical and Work Environment Requirements

• Ability to travel independently within the facility and community

• Ability to lift up to 50 lbs

• Manual dexterity and strength for performing wound care

• Strong focus, professional communication, and multitasking abilities

• Work setting includes nursing home, office, and patient care areas.
