Requirements:
Work collaboratively with RNCM Documentation Skills Communication Skills Ability to shift attention between tasks Ability to build trust with team/patients and family

Responsibilities:

• Collaborate effectively with the RN case management team to coordinate patient care.
• Provide documentation and communication support within the hospital setting.
• Assist in managing patient flow and ensuring a seamless care process.
• Support the healthcare team by adapting to patient needs and hospital policies.
• Maintain positive interactions with patients, families, and team members to ensure quality care.
