ED CARE NAVIGATOR

The Emergency Department (ED) Care Navigator serves as a key member of the interdisciplinary healthcare team, providing care coordination, patient navigation, and resource management services for patients presenting to the Emergency Department. The incumbent assesses patient needs, identifies barriers to care, facilitates access to healthcare and community resources, and supports safe transitions across the continuum of care. The ED Care Navigator collaborates with physicians, nursing staff, case management, social work, community agencies, and outpatient providers to improve patient outcomes, enhance patient engagement, and reduce avoidable healthcare utilization.
RESPONSIBILITIES
- Conduct screenings and assessments to identify barriers impacting access to care and successful health outcomes.
- Assist patients in navigating healthcare services, including primary care, specialty care, behavioral health, and community-based resources.
- Coordinate referrals and facilitate connections to internal and external support services.
- Support patients with scheduling appointments and obtaining necessary follow-up care.
- Provide education regarding available healthcare services, community resources, and treatment recommendations as directed by the healthcare team.
- Address social determinants of health including transportation, housing, food insecurity, financial assistance, and access to community resources.
- Maintain ongoing communication with patients and families to promote engagement in care plans and follow-through with recommended services.
- Collaborate with interdisciplinary teams to support care transitions and reduce barriers to accessing healthcare services.
- Document patient interactions, interventions, and outcomes in the electronic medical record and other required systems.
- Participate in quality improvement initiatives and population health programs.
- Maintain knowledge of community resources, healthcare programs, and available support services.
- Perform other duties as assigned.
QUALIFICATIONS/REQUIREMENTS
EXPERIENCE
- Minimum of two (2) years of experience in healthcare, human services, social services, customer service, community outreach, care coordination, or a related field preferred.
- Experience working with diverse populations and community resources preferred.
- Experience in a hospital, ambulatory care, public health, or community-based setting preferred.
EDUCATION
- Associates degree required.
- Bachelors degree in Human Services, Social Work, Public Health, Healthcare Administration, Psychology, Sociology, Community Health, or a related field preferred.
- An equivalent combination of education and experience may be considered.
LICENSES/CERTIFICATIONS
- Community Health Worker (CHW), Patient Navigator, or other related certification preferred.
SPECIAL REQUIREMENTS
- Strong communication, interpersonal, and customer service skills.
- Ability to build rapport and establish effective working relationships with patients, families, healthcare providers, and community partners.
- Knowledge of community resources and social service programs preferred.
- Proficiency with electronic medical records and Microsoft Office applications.
- Strong organizational skills with the ability to prioritize multiple responsibilities.
- Ability to maintain confidentiality and exercise sound judgment.
- Ability to work collaboratively as part of an interdisciplinary healthcare team.
