Broadly, the RN will also coordinate a resident’s overall care, while also presenting strategies to help self-manage chronic health conditions at home including diabetes, arthritis, and hypertension. The RN will also help DWC better understand the most prevalent types of chronic health conditions among our target population, and what kinds of additional support can be integrated into our Permanent Supportive Housing program to ensure that our residents are stable and able to safely age in place by addressing the chronic health and mental health challenges that pose threats to their stability.
