Schedule:

• Flexible shifts available Monday through Sunday
• Set your own hours to fit your lifestyle
• Commit to a minimum weekly or bi-weekly hourly requirement

Key Responsibilities:

• Complete annual health risk assessments in patients homes
• Review and document medical history, current health conditions, and medications
• Educate members about their health conditions and medications
• Identify care gaps and provide actionable recommendations for the care team
• Focus solely on assessmentno need to prescribe medication or create treatment plans

Qualifications:

• Active Nurse Practitioner License in Virginia
• MSN degree from an accredited institution
• Board certification through AANP or ANCC
• Current CPR (BLS) certification
• At least six months of NP experience is preferred
