Job Summary:
We seek a highly skilled and experienced Registered Nurse with a minimum of 2 years of acute care hospital experience, specializing in case management and discharge planning. The Registered Nurse will provide skilled nursing care, coordinate patient discharge plans, and collaborate with interdisciplinary teams to ensure optimal patient outcomes. This role requires strong communication and collaboration skills, with the ability to manage patient ratios effectively in a fast-paced acute care setting.
Responsibilities:

The Registered Nurse will provide skilled nursing care, coordinate patient discharge plans, and collaborate with interdisciplinary teams to ensure optimal patient outcomes. This role requires strong communication and collaboration skills, with the ability to manage patient ratios effectively in a fast-paced acute care setting.
Responsibilities:
- Conduct patient assessments and develop individualized care and discharge plans.
- Coordinate and manage case management activities including discharge planning and follow-up care.
- Collaborate with interdisciplinary teams, patients, and families to ensure continuity of care.
- Maintain accurate documentation and communicate patient status changes to relevant providers.
- Adhere to hospital policies and procedures, ensuring compliance with regulatory standards.
- Manage patient ratios of 1:25 on weekdays and 1:45 on weekends effectively.
- Participate in weekend shifts on a rotating basis to meet departmental needs.
- Active Registered Nurse license in California (pending license accepted if licensed by start date).
- Minimum of 2 years acute care hospital experience in case management or discharge planning.
- Strong clinical assessment, medication administration, and treatment planning skills.
- Excellent communication and interpersonal skills.
- Ability to work effectively in a fast-paced acute care environment.
- Experience with hospital case management ratios and discharge processes.
- Experience in behavioral health is not applicable; acute care hospital experience is mandatory.
- Previous travel nurse experience preferred but not required.
- No floating expectation; nurse will remain assigned to designated unit.
- Seasoned traveler required; first-time travelers are not accepted.
- Weekend work required every other weekend to meet department needs.
- Return to Office (RTO) restrictions limit hospital presence to no more than 7 days.
- Business attire or scrubs acceptable as dress code.
- Cancellation after signature may result in systemwide Do Not Return (DNR) status.
- Start date: ASAP.
- Work schedule includes weekdays with patient ratios of 1:25 and weekends with ratios of 1:45.
- Weekend shifts required every other weekend as per department needs.
- Shift pattern and hours determined by acute care facility standards.
