The Position:

We are looking for a passionate Home Health Admissions / Intake Coordinator to perform a wide range of duties including assessment of caller's inquiries, professional coordination, providing community relations, and maintaining effective interpersonal communication in a professional, friendly, and efficient manner. The Home Health Admissions / Intake Coordinator is responsible for the following duties:

• Coordination of an effective and efficient admission / intake process through referral processing, community partner collaboration, identifying the clinical team to deliver care, scheduling all patient visits within prescribed discipline and visit frequency requirements, insurance verification and authorization processing.
• Works directly with home health agency staff on facilitating admissions in a timely manner.
• Communicates all home health admissions / intake case assignments and schedules to members of the patient's treatment team

Requirements:

• High school diploma or equivalent.
• Associate degree in Healthcare and/or prior relevant work experience preferred
• A track record of successful experience in a health care setting, strong interpersonal skills, and experience working with physicians, nursing management, discharge planners, case managers, and social workers.
• Ability to examine record and verify patient identification. Knowledge of standard medical terminology, basic human anatomy and medical record format, content and medical documentation order preferred
• Experience in customer service, call center, administrative role, or related area.
• Two years of related experience in admissions role and/or home health preferred
