Location Aviva Independent Living - Port Saint Lucie, FL Responsibilities Conduct initial evaluations to assess patients' needs and develop individualized treatment plans Develop and implement appropriate treatment plans based on patient needs and goals Monitor patient progress and adjust treatment plans as necessary Educate patients and caregivers on proper techniques to improve mobility, reduce pain, and prevent injuries Maintain accurate and timely documentation of patient progress Collaborate with other healthcare professionals to ensure patients receive comprehensive care Provide home health and/or outpatient services as needed, including supervisory visits, to ensure optimal outcomes Qualifications A degree in Occupational Therapy from an accredited program Must be licensed or registered by the applicable State without restrictions. Must have knowledge of Medicare and Medicaid guidelines Must have a valid driver’s license and reliable transportation. Must have a current CPR certification. Excellent verbal and written communication skills. Basic computer skill proficiency. Ability to use Microsoft Windows. Background Screening Requirement Employment in Florida is contingent upon completion of a Level 2 background screening in accordance with Florida AHCA requirements. Applicants will be required to complete fingerprinting and background screening through the AHCA Clearinghouse.
