Key Responsibilities\n

  • Conduct thorough, ongoing assessments to identify and address the unique mental health needs of each patient.

  • Maintain open lines of communication with supervisors, family members, and treatment teams regarding client progress and required interventions.

  • Create and share a detailed tentative schedule with supervisors outlining appointments, home visits, and other necessary activities.
  • \n
  • Coordinate and perform home visits to assess client environments and integrate findings into individualized treatment plans.
  • \n
  • Ensure the accurate and effective implementation of treatment plans while documenting client progress and outcomes.
  • \n
  • Participate actively in interdisciplinary team meetings to provide insights and updates on client statuses.
  • \n
  • Complete monthly and quarterly progress notes to track treatment efficacy and areas for improvement.
  • \n
  • Assist in organizing and delivering training sessions for staff to ensure best practices in program implementation.
  • \n
  • Conduct follow-up interviews and continued supportive home visits to assess ongoing client needs and adjustments to care.
  • \n
  • Act as a liaison with community resources and the mental health sector to foster collaborative relationships and support community development goals.

Patient Population / Client Focus\n

In this role, you will work with a diverse range of clients, including adults and youth facing various mental health issues such as anxiety, depression, trauma, and substance misuse. Your expertise will be crucial in providing the support and therapeutic interventions necessary for their recovery and overall wellbeing.

Qualifications\n

  • A Master’s or Doctoral degree in Counseling, Psychology, or a closely related field.
  • \n
  • Possession of an active and unrestricted LPC license in the state of Arizona.
  • \n
  • A minimum of two years of experience in counseling, preferably in a clinical or community-based setting.

  • Strong familiarity with legal and ethical standards in mental health practice.
  • \n
  • Exceptional interpersonal and communication skills to establish rapport and build trust with clients from varied backgrounds.

  • Experience collaborating with a multidisciplinary healthcare team to ensure comprehensive client care.

  • Proficient in utilizing electronic medical record (EMR) software and related technological tools to document and track patient care.

Preferred Experience\n

  • Specialized training or experience in family therapy, couples therapy, trauma-informed care, or substance abuse interventions is highly preferred to address the wide spectrum of client needs.

  • Competitive salary range of USD 90,000 to 100,000 annually, commensurate with experience and caseload.
