Afterhours Hospice Care Manager RN

Heartland Hospice is seeking an Afterhours RN Care Manager to provide high-quality hospice care outside standard business hours. This role ensures coverage, continuity of care, and clinical oversight for patients and families during nights, weekends, and on-call hours. The RN assesses, coordinates patient and family needs, and collaborates with the IDG to align plans of care.
