Home Health RN Case Manager - Chickasha, OK!

Join our compassionate and dedicated team at Haven Home Health and Hospice as a full-time RN Case Manager!We are seeking an experienced, committed Registered Nurse to provide direct patient care and coordinate services in accordance with each patient’s individualized plan of care. This role involves travel throughout the OKC Metro area and is based out of our main office in Norman, OK.Responsibilities:Deliver skilled nursing visits to assigned patients based on physician-ordered plan of careDevelop and regularly update individualized care plansComplete and document comprehensive assessments, including:Start of Care (SOC)Resumption of Care (ROC)RecertificationDischarge assessmentsSupervise and support Home Health AidesEducate patients and caregivers on disease processes, medications, and rehab goalsAttend and lead weekly case conferences for collaborative care planningCoordinate care with physicians and interdisciplinary team membersReport any changes in patient condition to appropriate personnel promptlyEnsure timely, complete, and accurate documentation that aligns with Medicare and agency guidelinesAdminister medications and treatments as prescribedParticipate in on-call, holiday, and weekend rotationsAssist with orientation and training of new staffMaintain compliance with all HIPAA and agency confidentiality standardsQualifications:Current RN license in the state of Oklahoma (Required)Minimum 1 year of Home Health experience (Preferred)Strong assessment and communication skillsExperience with electronic medical records preferredValid driver’s license, reliable transportation, and current auto insuranceAbility to travel in varying weather conditionsMust be able to travel to our main office in Norman, OKSchedule:Monday to FridayFlexible ShiftOn-call rotation once a month requiredCompensation & Benefits:Competitive salaryMileage reimbursementHealth, Dental, and Vision InsuranceSupplemental BenefitsPaid Time Off (PTO)Flexible schedulingWe’re looking for someone who is compassionate, self-motivated, and committed to delivering exceptional care.
