Patient Educator RN or RD - Diabetes Education

This role would support multiple locations: schedule will likely be 1 day/week in Aurora, 2 days in Glen Ellyn and 2 days in Geneva. Employees will be assigned specific days to cover inpatients at CDH (Winfield) and Delnor (Geneva) for inpatient consults.

The Patient Educator understands and reflects the mission, vision, and values of NM, adheres to the organization's Code of Ethics and Corporate Compliance Program, abides by HIPAA and complies with all relevant policies, procedures, guidelines and all other regulatory and accreditation standards.

Responsible for assessing, planning, implementing and evaluating educational activities for the assigned service.

Responsibilities:

• Requires critical thinking skills, decisive judgment and the ability to work with minimal supervision. Must be able to work in a stressful environment and take appropriate action.
• Collaborates with nurse managers and staff to assess the learning needs and competencies of staff.
• Develops, implements and evaluates educational plans/programs and utilizes methods, measurements and tools to evaluate achievement of learning goals.
• Serves as a consultant for staff in areas of program development, clinical practice, and professional development.
• Participates in the coordination and continuity of orientation programs, including preceptor programs.

Qualifications

Required:

• Registered RN or Dietitian in the State of Illinois.
• Master's degree in Nutrition or Dietetics
• BLS Healthcare provider required or within 90 days of hire.

Preferred:

• CBDCE Certification from the Certification Board for Diabetes Care and Education
