The Westchester Institute for Human Development seeks an experienced Registered Nurse Care Coordinator and Clinic Floor Nurse to support outpatient services, coordinate chronic care management, and help ensure high-quality patient outcomes. In collaboration with the Nurse Manager, providers, and clinical staff, you will perform triage, administer interventions, manage care plans, arrange referrals and follow-ups, and contribute to infection control and quality improvement efforts. #J-18808-Ljbffr Westchester Institute For Human Development
