Medical Social Worker

Provide individualized medical social work care to adult and geriatric home health patients under the supervision of a Clinical Manager Assess, interpret, plan, implement, and evaluate social and emotional factors related to patients’ illnesses Provide goal-oriented therapy, counseling, psychotherapy, crisis intervention, and conflict resolution Strengthen family support and help patients and caregivers cope with medical conditions and treatment Assess psychosocial and emotional needs using professional assessment and interview techniques Educate patients and families about available resources and make community resource referrals Supervise social work assistants and review their work Manage social work needs referred by other disciplines as part of a multidisciplinary team Complete forms and orders accurately and maintain patient records, caseloads, and census Attend interdisciplinary team meetings and coordinate services with physicians and staff Report changes in patient condition, safety issues, care-plan changes, discharge plans, concerns, and protective-services referrals Participate in program development, student programs, performance improvement, community outreach, and professional development Maintain and adjust schedules, meet productivity expectations, and assist team members Follow confidentiality, ethics, safety, OSHA bloodborne-pathogen, and agency policies Promote customer satisfaction and demonstrate the agency’s mission, vision, and values Requirements Master of Social Work (MSW) from an accredited school of social work Certified (licensed) or the ability to be certified as a Clinical Social Worker by the State Certification Board for Social Work One year of experience as a Clinical Social Worker in a healthcare setting Home health experience preferred Ability to speak clearly and communicate with patients, families, physicians, and staff Must be able to communicate and be literate in the English language Hand/eye coordination to use a computer Ability to perform basic patient transfers and guard patients Ability to assess a patient’s psychosocial situation, formulate treatment plans, select appropriate social work interventions, evaluate responses to treatment, and engage patients in treatment Ability to prioritize duties, learn new skills and techniques Ability to visually assess patients and use hearing to communicate effectively Must have a valid driver’s license and an operational vehicle Able to sit, write, walk, climb stairs, stand, and drive an automobile Ability to withstand dust, smoke, fumes, bloodborne pathogen exposure, body fluids, dangerous animals, traffic hazards, weather extremes, and potential personal-safety threats Essentially 40 hours per week, with schedule flexibility to accommodate patient needs Core Competencies Demonstrates expertise in providing individualized medical social work care, including assessment, planning, and implementation of treatment plans for adult and geriatric patients.
