Position Overview:
Seeking a skilled and compassionate Registered Nurse to provide high-quality home health services to patients within the Gwinnett coverage area. This role focuses on delivering in-home skilled nursing visits, completing comprehensive assessments, and supporting patient care plans to promote optimal health outcomes. The ideal candidate brings strong clinical judgment, home health experience, and the ability to work independently in a patient-focused environment. Schedule & Work Details:

The ideal candidate brings strong clinical judgment, home health experience, and the ability to work independently in a patient-focused environment. Schedule & Work Details:
- Location: Gwinnett County, GA
- Schedule: Monday through Friday, 8:00 AM to 4:30 PM
- Weekly Hour Guarantee: 32 hours
- On-call/Weekend Requirement: Yes, once per month
- Pay Rate: $51 to $53 per hour
- Mileage Reimbursement: Yes, from first patient to last patient
- Provide skilled nursing visits in patient homes in accordance with physician orders and plan of care
- Complete a minimum of 30 productivity points per week
- Perform comprehensive patient assessments, including OASIS documentation
- Develop, implement, and update individualized care plans
- Monitor patient progress and communicate changes to physicians and care team
- Educate patients and families on disease management, medications, and care plans
- Ensure accurate, timely documentation within Homecare Homebase (HCHB)
- Maintain compliance with agency policies, state regulations, and clinical standards
- Active Registered Nurse (RN) license in the state of Georgia
- OASIS experience required
- Homecare Homebase (HCHB) experience required
- Strong clinical assessment and critical thinking skills
- Ability to work independently and manage time effectively
- Previous home health experience
- Demonstrated strong assessment abilities and clinical skills
- Excellent communication and patient education skills
- Adaptability and commitment to patient-centered care
