Responsibilities

• Conduct patient assessments to identify nutritional needs and develop personalized meal plans

• Collaborate with healthcare team members to ensure effective communication and coordination of care

• Provide nutritional counseling and education to patients and their families

• Monitor patient progress and adjust nutrition plans as needed

• Develop and implement nutrition programs to promote patient well-being and quality of life

• Participate in quality improvement initiatives to enhance patient outcomes and satisfaction

• Maintain accurate and timely records of patient care and nutrition plans

Requirements

• Bachelor's degree in Nutrition or Dietetics

• Registered Dietitian (RD) credential

• Experience in acute care, rehabilitation, or long-term care settings (2 years of experience minimum)

• Strong communication and interpersonal skills

• Ability to work effectively in a fast-paced environment
