Responsibilities

• Evaluate and assess patients' functional limitations across acute, swing, skilled nursing, and outpatient environments.
• Develop and implement individualized rehabilitation plans focused on restoring mobility, strength, and independence.
• Deliver treatments ranging from therapeutic exercises to neurological and orthopedic interventions.
• Monitor patient progress and adjust plans of care as needed to optimize outcomes.
• Collaborate with physicians, nurses, and occupational therapists to ensure integrated, patient-centered care.
• Educate patients and families on therapy goals, recovery strategies, and injury-prevention techniques.
• Document care thoroughly and comply with HIPAA and regulatory standards.
• Contribute to department operations, quality assurance, and continuing education initiatives.
• Supervise assistants and mentor new team members when applicable.

Qualifications

• Current Physical Therapy license from the State of California.
• Current BLS certification (AHA required).
• Minimum one year of experience in acute care, extended care, rehabilitation, or outpatient setting.
• Strong interpersonal, communication, and documentation skills.
• Passion for working across diverse patient populations (orthopedic, neurological, post-surgical, geriatric, etc.).
