Required Qualifications

Active LPN License (Illinois)
2+ years of Skilled Nursing Facility (SNF) experience

Key Responsibilities

Provide high‑quality nursing care to SNF residents
Administer medications and treatments accurately
Monitor patient conditions and report changes promptly
Collaborate with RNs, CNAs, and interdisciplinary teams
Maintain thorough and compliant documentation
