Physical Therapist - Travel Contract

Traveling Physical Therapist Opportunity

Looking for a change of scenery and a chance to help others. Join a home health team in Jeffersonville, IN as a traveling Physical Therapist where you will help patients recover from injuries and manage chronic conditions while providing care in patients' homes. You will perform comprehensive evaluations, develop measurable goals and treatment plans, document according to required formats and timelines, and coordinate care with physicians, families, and the interdisciplinary team. Supervision of PT Assistants and aides as allowed by state practice act will be required. Maintain required continuing education and adhere to professional and regulatory standards.

Minimum Qualifications

• Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA.
• Active Indiana physical therapy license required to start the assignment.
• Current BLS certification from AHA or ARC may be required.
• Home health clinical experience required.
• Experience with HCHB (Homecare Homebase) and OASIS documentation required.
• Ability to cover assigned counties including Washington, Scott, partial Floyd, and partial Clark as part of the patient census and travel expectations.
• Experience performing comprehensive PT evaluations, developing measurable goals, and creating individualized treatment plans.
• Documentation and billing experience compliant with company, state, and federal regulations.

Preferred Qualifications

• Previous travel or per-diem home health experience preferred.
• Experience supervising PT Assistants and aides preferred.
• Familiarity with productivity point systems preferred (example: 30 points).

Assignment Details

• Shift Requirements: Monday through Friday, 8:00 AM to 5:00 PM.
• Minimum of one weekend a month on call.
• Counties covered: Washington, Scott, partial Floyd, and partial Clark.
• Mileage reimbursement: $0.46 per mile.
