Position Summary

We are seeking an experienced Clinical Manager to oversee patient care, supervise clinical staff, and lead Quality Assessment and Performance Improvement (QAPI) activities. This role ensures care is coordinated effectively, services are delivered appropriately, clinical documentation meets regulatory standards, and the team is supported in providing high-quality, patient-centered care.

Key Responsibilities

• Review new referrals and patient information to determine clinical needs and appropriateness for services.
• Assign appropriate clinical personnel and coordinate admissions with the Medical Director and interdisciplinary team.
• Supervise, coach, and support clinical staff to promote strong performance, accountability, and high-quality patient care.
• Review patient medical histories, diagnoses, medications, procedures, care plans, and clinical progress.
• Assist clinicians with establishing therapeutic goals, care priorities, and individualized plans of care.
• Participate in case conferences and interdisciplinary team meetings to support coordinated care.
• Conduct quarterly clinical record reviews and provide feedback, education, and corrective guidance to staff and leadership.
• Support the recruitment, onboarding, orientation, and ongoing development of clinical staff.
• Plan and assist with staff training, continuing education, competency assessments, and implementation of organizational policies.
• Lead and coordinate the organization's QAPI program in collaboration with leadership.
• Identify quality concerns, performance gaps, trends, and opportunities for improvement.
• Collect, review, and analyze clinical, operational, complaint, incident, and performance data.
• Develop, implement, monitor, and document performance improvement plans and quality initiatives.
• Track QAPI outcomes and prepare reports for leadership, governing body meetings, and regulatory reviews.
• Follow up on corrective action plans to ensure improvements are implemented and sustained.
• Support survey readiness and ensure compliance with federal, state, accreditation, and organizational requirements.
• Assess community health needs and assist with developing programs and services to address identified needs.
• Promote the organization's role within the community and support a patient- and customer-service-focused culture.
• Complete monthly billing workflows and other duties delegated by leadership.
• Ensure clinical practices remain consistent with professional standards, hospice philosophy, and regulatory requirements.

Qualifications

• Current and unrestricted Registered Nurse license in the applicable state.
• Bachelor of Science in Nursing Required
• Previous hospice or home care experience required.
• Minimum of two years of clinical supervisory or management experience preferred.
• Previous experience with QAPI, quality improvement, compliance, audits, or performance improvement activities strongly preferred.
• Strong understanding of hospice philosophy, pain and symptom management, and end-of-life care.
• Knowledge of clinical documentation standards and applicable federal, state, and accreditation requirements.
• Strong clinical judgment, observation, organization, leadership, and communication skills.
• Ability to analyze quality data, identify trends, and implement effective corrective action plans.
• Proven ability to work effectively within an interdisciplinary team.
• Valid driver's license with an insured and reliable vehicle.
• Current CPR certification.

Pay Range for Posted Region

$100,000 - $120,000 USD
