Occupational Therapist (OT)

Overview Home Health Occupational Therapist (OT) in Sacramento – a role that customizes therapy to a patient’s life and optimizes outcomes. As a Home Health OT, you’ll help patients regain independence and learn to perform everyday activities in the comfort and familiarity of home. Responsibilities Provide occupational therapy to patients unable to perform daily tasks due to illness or injury. Work as part of a home‑health team that may include an RN, LPN, CNA, HHA, PT, and SLP, focused on the patient’s plan of care and personal goals. Assess patients, observe deficits, establish therapy goals, and document progress. Assist patients with exercises to improve fine motor skills and coordination. Suggest adaptive equipment such as grab bars and shower chairs to offer added support. Assess fall risks and introduce strategies to improve home safety. Educate patients and families on plan of care, exercises, goals, and self‑care. Qualifications Graduate of an accredited Occupational Therapy Program. Active OT license in California. Minimum of 1 year of occupational therapy experience, ideally in home healthcare. CPR certification (and any other relevant certifications). Knowledge of state and federal home health regulations.
