We are currently seeking a motivated and compassionate Outreach Specialist to join our growing team. This role is ideal for someone who enjoys building relationships, educating others about available healthcare services, and helping patients and families connect with the support they need. As an Outreach Specialist with Trusted Nursing, you will serve as an important liaison between our organization, patients, families, healthcare providers, and community partners. You will help expand awareness of our services while ensuring individuals eligible for care receive the information and guidance necessary to access support. Why Work With Trusted Nursing? Key Responsibilities Responsibilities of the Outreach Specialist may include: Building and maintaining relationships with referral sources, healthcare providers, community organizations, and patient advocates Educating patients, families, and community partners about Trusted Nursing services and EEOICPA benefits Conducting outreach efforts within the community to increase awareness of available healthcare services Assisting prospective patients and families through the intake and referral process Coordinating communication between patients, caregivers, office staff, and clinical teams Following up with referrals and maintaining consistent communication with referral sources Attending community events, meetings, health fairs, and networking opportunities as needed Supporting patient recruitment and census growth initiatives Maintaining accurate records of outreach activities, referrals, and communications Assisting with marketing and community engagement efforts Ensuring professionalism, confidentiality, and compassionate communication at all times Collaborating with leadership and clinical staff to identify opportunities for growth and improved patient outreach Responding promptly to inquiries from patients, families, and referral partners Assisting with administrative tasks related to outreach and patient coordination Qualifications Previous outreach, healthcare marketing, admissions, customer service, case management, or community relations experience preferred Healthcare or home health experience preferredStrong communication and relationship-building skills Professional and compassionate demeanor Ability to work independently and manage multiple priorities effectively Strong organizational and time management skills Comfortable speaking with patients, families, healthcare professionals, and community partners Basic computer and documentation skills required Ability to maintain confidentiality and professionalism in all interactions Reliable transportation and valid driver’s license required Must be able to read, write, and communicate effectively in English Must pass a background check Benefits Base salary and several commission opportunities Flexible scheduling opportunities Supportive and collaborative team environment Opportunity to make a meaningful difference in patients’ lives Professional growth and advancement opportunities Community-focused and relationship-driven work environment Rewarding work serving individuals who sacrificed for our country Our patients cared for our country — now it’s our turn to care for them.
