Hospice RN Case Manager - Compassionate In-Home Care

Gentiva

Gentiva is seeking a Hospice Registered Nurse (RN) Case Manager to guide patients and families through end-of-life care in their homes across Kansas. You will provide skilled nursing care, perform assessments, and collaborate with an interdisciplinary team to develop and update personalized plans of care.

You will educate families, manage symptoms, and ensure accurate documentation while adhering to regulatory standards and infection control guidelines.
