Case Manager — Drive Housing Stability & Social Impact

PATH (People Assisting the Homeless)

PATH (People Assisting The Homeless) is hiring a Case Manager for the Long Beach, CA location. This full-time role supports clients through housing navigation, healthcare access, and service coordination within the Time Limited Subsidy program. You will conduct intake, develop individualized plans, provide referrals, and maintain detailed case files while upholding HIPAA and PATH policies.
