This is a telehealth-based Internal Medicine role operating on-site from the practice's Brooklyn location — a meaningful, high-impact position serving a vibrant and diverse inner-city population. Position Overview: US Career Partners is recruiting on behalf of a well-established community outpatient medical center in Canarsie, Brooklyn, seeking a personable, driven, and compassionate Internal Medicine Physician to join their team in a telehealth capacity, operating on-site from the Brooklyn practice. This role is well-suited for a physician who is energized by high-volume, high-impact work and finds deep satisfaction in serving an inner-city population with diverse and complex medical needs. Responsibilities:

• Provide comprehensive internal medicine care via telehealth, conducted on-site from the Brooklyn practice location
• Manage a broad range of acute and chronic medical conditions including diabetes, hypertension, cardiovascular disease, and related disorders
• Conduct virtual patient evaluations, develop individualized treatment plans, and ensure appropriate follow-up care
• Collaborate seamlessly with an integrated in-house team of specialists across more than 30 departments
• Coordinate internal referrals and ensure continuity of care for patients with complex medical needs
• Educate patients on disease management, preventive care, and healthy lifestyle practices
• Maintain thorough, accurate, and up-to-date medical documentation in accordance with established standards
• Adhere to all applicable Medicaid, regulatory, and ethical standards of medical practice

Qualifications:

• Medical degree from an accredited institution (MD or DO)
• Board Certification or Board Eligibility in Internal Medicine
• Active New York Medicaid provider number — required
• Strong clinical acumen across a broad range of adult medical conditions
• Warm, engaging personality with a genuine passion for serving diverse, inner-city communities
• Ability to work effectively in a high-volume, fast-paced outpatient telehealth environment
• Excellent interpersonal and communication skills
• Demonstrated ability to collaborate within a team-based, multi-specialty care model
