Managed Care Coordinator

Location Must reside within Richmond, Chesterfield, or Central Virginia area. This field-based role enables associates to primarily operate in the field, traveling to client sites or designated locations as required, with occasional office attendance for meetings or training. Alternate locations may be considered if candidates reside within a commuting distance from an office. How You Will Make An Impact Perform telephonic or face-to-face history and program needs assessments using a pre-defined tool to identify, evaluate, coordinate, and manage members’ program needs. Identify members with potential clinical health-care needs (including high-risk complications) and coordinate those cases with the clinical health-care management and interdisciplinary team to provide care coordination support. Manage non-clinical needs of members with chronic illnesses, co-morbidities, and/or disabilities to ensure cost-effective and efficient utilization of services. Establish short and long-term goals in collaboration with the member, caregivers, family, natural supports, and physicians. Identify members that would benefit from expanded services. Travel to worksite and other locations as necessary. Minimum Requirements BA/BS degree and a minimum of 1 year of experience working directly with people related to the specific program population or other community-based organizations; or an equivalent combination of education and experience. Preferred Skills, Capabilities, And Experiences BA/BS degree in a healthcare-related field preferred. Specific education, years, and type of experience may be required based on state law and contract requirements. For MMP, requires Community Health Worker certification to be obtained within one year of employment.
