SNF PT Opportunity – Clermont, FL

Ardor Health Solutions is seeking a Physical Therapist for a 13-week Skilled Nursing Facility travel assignment in Clermont, Florida .

This is a full-time rehabilitation opportunity working primarily with adult and geriatric patients. You'll help patients regain strength, balance, mobility, safety, and functional independence following hospitalization, surgery, illness, or injury.

Qualifications

• Graduate of an accredited Physical Therapy program
• Current Florida PT license or ability to obtain prior to start
• Current BLS certification
• Strong evaluation and treatment skills
• Comfortable treating adult and geriatric patients
• SNF or geriatric rehabilitation experience preferred
• Strong documentation and communication skills

What You'll Do

• Complete PT evaluations and reassessments
• Develop individualized plans of care
• Provide gait and balance training
• Address strength, endurance, transfers, mobility, and functional limitations
• Provide therapeutic exercise and functional training
• Educate patients and caregivers
• Provide fall-prevention and safety education
• Collaborate with PTA, OT, COTA, SLP, nursing, and other members of the care team
• Assist with discharge planning and recommendations
• Complete timely clinical documentation
