About the Role:

We are seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our night shift team at our Bradford Manor, Pennsylvania, facility. As an LPN at Bradford Manor Nursing and Rehabilitation , you will play a vital role in delivering exceptional patient care and contributing to our mission of providing high-quality services to our patients.

Responsibilities:

Provide direct patient care under the supervision of a registered nurse, including administering medications, dressing wounds, and performing vital signs checks
Develop and implement individualized care plans for patients, in collaboration with the interdisciplinary care team
Maintain accurate and detailed records of patient care and progress
Collaborate with other healthcare professionals to ensure seamless transitions of care and effective communication
Participate in quality improvement initiatives to enhance patient outcomes and satisfaction

Requirements:

Current LPN license in the state of Pennsylvania
Basic Life Support (BLS) certification
