Job Summary: We are seeking a friendly and compassionate Cardiology Nurse Practitioner/ Physician Assistant to join our growing team. In this role, you will provide comprehensive pulmonary care to patients in post-acute care settings while collaborating closely with physicians, nursing staff, and interdisciplinary care teams. Responsibilities: Conduct pulmonary assessments for patients in post-acute care settings Evaluate, diagnose, and manage cardiovascular conditions including hear failure, hypertension, coronary artery disease (CAD), congestive heart failure (CHF), arrhythmia, valvular heart disease, and prior myocardial infarction (MI). Develop and implement individualized treatment plans Collaborate with interdisciplinary teams Educate patients, families, and facility staff on cardiovascular conditions, cardiac treatment plans, disease prevention, medication management, and continuity of care. Monitor patient progress and adjust care plans as needed Maintain accurate and timely EMR documentation Support initiatives focused on reducing hospitalizations and Return-to-Hospital (RTH) events Requirements Preferred a minimum of 6 months of experience in post-acute care setting Preferred experience in Cardiology: training provided Active and unrestricted state Nurse Practitioner license or Active PA license and NCCPA certification required Master s Degree in Nursing from an accredited Nurse Practitioner program or Master s Degree from an accredited Physician Assistant program required.
