Registered Nurse (RN) - Bronson At Home (Home Health) - PRN Location: BAH Bronson at Home Shift: First Shift Time Type: Part time Scheduled Weekly Hours: 10 Responsibilities Provide skilled nursing services to home care clients according to physician‑prescribed plans of care and Medicare Conditions of Participation. Work as part of a geographic or specialty team under the direction of the HH Clinical Services Manager. Utilize the nursing process—assessment, planning, diagnosing, implementation, and evaluation—to achieve quality outcomes. Coordinate care with other professionals and delegate tasks within the scope of nursing practice. Provide selected physical care within scope of practice based on patient acuity and complexity. Document each visit, anticipate needs, and communicate effectively with disciplines. Perform assessments, develop care plans, implement interventions, administer medication, educate patients, and evaluate progress. Manage care plans, organize and delegate interventions, evaluate effectiveness, and document outcomes. Participate in quality improvement, educational activities, and professional development. Maintain professionalism, confidentiality, ethical practice, and patient advocacy. Assist patients and caregivers in informed decision‑making. Lead and collaborate with health disciplines to achieve positive outcomes. Qualifications Diploma, Associate’s, or Bachelor’s degree in Nursing. Licensed RN in good standing with the State of Michigan. Two years of current continuous experience in an acute care setting (preferred). Current driver’s license in Michigan and proof of auto insurance. BLS certification. Own vehicle for travel; must pass an MVR check and maintain valid insurance. Ability to read, interpret, and analyze data from computer systems. Excellent verbal, written, and electronic communication skills. Physical ability to lift up to 50 pounds and perform repetitive, detail‑oriented work. Commitment to nursing professional standards, quality improvement, and ethical practice. #J-18808-Ljbffr
