Annexia Home Health is hiring a Certified Occupational Therapy Assistant (COTA) to join our growing home health team in Federal Way, WA. About the Role The Certified Occupational Therapy Assistant (COTA) provides occupational therapy services to patients in their homes under the direction and supervision of a Washington-licensed Occupational Therapist (OT). The COTA implements the established occupational therapy plan of care, provides appropriate interventions within the COTA’s scope of practice, monitors patient response and progress, provides patient and caregiver education, and communicates clinical findings and changes in patient condition to the supervising OT and interdisciplinary team. The COTA is also responsible for completing accurate and timely clinical documentation and following applicable professional, regulatory, payer, and agency requirements. Key Responsibilities Provides occupational therapy interventions in accordance with the established plan of care and the COTA’s scope of practice. Implement therapeutic exercises, activities, adaptive techniques, and other interventions as directed by the supervising OT. Provide individualized treatment designed to improve or maintain patients’ functional abilities, independence, safety, and participation in activities of daily living. Monitor and document patient response to treatment and progress toward established goals. Communicates changes in patient condition, functional status, response to treatment, or other clinical concerns to the supervising OT and appropriate members of the care team. Provide patient and caregiver education related to safety, therapeutic activities, adaptive techniques, home programs, and equipment as appropriate. Assist with the selection, fitting, and training of patients in the use of adaptive equipment and self-help devices as directed by the supervising OT. Performs visits in patients’ homes and maintains a professional and respectful environment. Completes accurate, timely, and clinically appropriate documentation for all services provided. Participates in case conferences, interdisciplinary communication, in-services, and other meetings as required. Maintains regular, documented consultation with the supervising OT in accordance with applicable Washington requirements and Annexia Home Health policies. Reports patient safety concerns, changes in condition, and other significant findings promptly. Maintains patient confidentiality and complies with HIPAA and applicable federal and state requirements. Follows Annexia Home Health policies, procedures, infection prevention practices, and professional standards. Performs other duties consistent with the position and professional scope of practice as assigned. Qualifications Required Qualifications: Current Washington State license as an Occupational Therapy Assistant (OTA). Current BLS certification from an approved provider, or ability to obtain certification by the required deadline. Valid driver’s license. Reliable transportation for patient visits. Current automobile insurance meeting applicable state and agency requirements. Ability to travel to patients’ homes throughout the assigned service area. Strong verbal, written, organizational, and interpersonal communication skills. Ability to work independently while maintaining effective communication with the supervising OT and interdisciplinary team. Preferred Qualifications: Minimum of one (1) year of home health occupational therapy assistant experience. Experience working with Medicare and Medicaid populations. Experience working with older adults and/or patients with multiple chronic conditions. Familiarity with electronic medical records and home health documentation. Bilingual abilities (Spanish or other languages prevalent in the region).
