RN Case Manager
Good Samaritan
Good Samaritan Health Center seeks an RN Case Manager to coordinate holistic care for underserved patients in our faith-based, mission-driven clinic. The RN Case Manager conducts assessments, develops and manages care plans, supports care transitions, and connects patients with community resources. Working closely with an interdisciplinary team, this role promotes health equity, patient education, and whole-person wellbeing. Ideal candidates bring strong clinical judgment, case management expertise, and a passion for serving diverse, low-income communities in a collaborative, faith-informed environment.
