LPN Extended Care

Job Summary:

The Licensed Vocational Nurse (LPN) providing care to patients in Skilled Nursing/Extended Care and SCU settings is responsible for assessing, planning, implementing, and coordinating patient care. The role includes delivering nursing care to the patient population while collaborating with healthcare teams to ensure quality outcomes. This position requires night shift work with on-call responsibilities and potential overtime. Responsibilities:

• Assess, plan, implement, and coordinate patient care in Skilled Nursing/Extended Care and SCU units.
• Provide nursing care as directed by policies and procedures.
• Gather and report patient data accurately.
• Carry out physician and nursing orders.
• Perform therapeutic procedures appropriate to the patient's condition and age.
• Collaborate with interdisciplinary team members to optimize patient outcomes.
• Maintain accurate and timely documentation of patient care activities.
• Participate in on-call rotations and respond to patient needs during those shifts.

Required Skills & Certifications:

• Current LPN license valid in the state of Colorado.
• Basic Life Support (BLS) certification.
• Experience in inpatient or extended care nursing preferred.
• Strong clinical assessment and patient care skills.
• Ability to work independently and as part of a healthcare team.

Preferred Skills & Certifications:

• Experience working in emergency, critical care, or skilled nursing environments.
• Comfort and familiarity with ventilators, tracheostomies, and PICC lines.
• Additional certifications or advanced training related to extended care nursing.

Special Considerations:

• Night shift position with 12-hour shifts.
• On-call responsibilities required approximately 2 shifts every 6 weeks.
• Potential for overtime as needed.
• Shared furnished employee housing may be available with restrictions (no pets, partners, or smoking).
• Work locations include Kremmling, CO, with possible coverage in Granby MedSurg.

Scheduling:

• 12-hour night shifts.
• Shift schedule includes on-call rotation.
• Overtime availability may be required based on patient care needs.
