As a Social Worker/Care Companion, you will have the opportunity to make a meaningful difference in the lives of older adults by providing emotional support, brief therapeutic interventions, and mental health assessments to residents in skilled nursing facilities. Key Responsibilities Visit designated facilities in accordance with the schedule set with your supervisor Conduct assessments of residents to identify emotional, cognitive, and psychosocial needs Implement short‑term, solution‑focused therapy and supportive interventions to address immediate concerns Document visit notes and maintain detailed records of interactions and interventions after each visit Collaborate with psychiatric professionals to ensure comprehensive care, including participation in a monthly meeting with the psychiatric NP Qualifications Associate’s degree in Social Work, Psychology, Counseling, or another mental‑health related field Understanding of geriatric mental health, crisis management, and psychosocial support in long‑term care settings Strong communication, documentation, and relationship‑building skills Minimum 1 year of experience Benefits Meaningful, resident‑focused work in a supportive team environment Regular supervision and collaboration with psychiatric providers Professional growth opportunities through monthly clinical meetings and ongoing training Opportunity to bring comfort, connection, and care to residents who need it most Flexible scheduling: approximately 15 hours per week, Monday‑Friday, 8‑5 hours Paid training and ongoing support from our clinical team Matching 401 k plan Join our team and start making a difference today!
