Position Overview

This role is dedicated to delivering clinical and palliative care services within hospital or homecare environments. The practitioner supports the assessment, development, implementation, monitoring, and evaluation of clinical programs and policies. They actively contribute to quality improvement initiatives and collaborate closely with physicians and healthcare teams to optimize patient outcomes and resource management.

Key Responsibilities

• Maintain a safe and clean work environment, adhering to unit safety and infection control protocols.
• Provide direct clinical services in hospital settings.
• Monitor patient outcomes, identify barriers to care, and collaborate with interdisciplinary teams to address challenges.
• Educate physicians, inpatient staff, and the community on hospice and end-of-life care services.
• Participate actively in performance improvement programs.
• Oversee clinical service assessments, development, and evaluations.
• Conduct patient health assessments through histories and examinations.
• Refer patients to alternate care when necessary.
• Coordinate with infection control departments on reporting, screening, and prevention programs.
• Prepare and present monthly reports and statistics to management.
• Develop and review clinical policies, procedures, and monitoring tools.
• Continuously improve quality of care and service delivery.
• Collaborate with medical directors, physicians, and healthcare professionals to ensure effective clinical service provision.

Qualifications

• Minimum of 2 years experience as an adult Nurse Practitioner; candidates with less NP experience but at least 5 years of RN acute care experience may be considered.
• Completion of all Nurse Practitioner clinical requirements.
• Proficient computer skills.
• Strong customer service and communication abilities.
• Bilingual in English and Spanish is highly preferred.

Education

• Graduate of an accredited nursing school.
• Master of Science in Nursing (MSN) degree required.

Licenses and Certifications

• Current Registered Nurse (RN) license.
• Current Nurse Practitioner license.
• Basic Life Support (BLS) certification from the American Heart Association required upon hire.

Employment is contingent upon passing a physical exam, drug screening, and background checks. Compliance with regional COVID-19 vaccination mandates is required.
