Hospice Care Sales Representative

Heartland Hospice, part of ProMedica Hospice, is seeking a Hospice Care Consultant to grow and support our hospice services. This role focuses on educating physicians, facilities, patients, and families about hospice, building referral relationships, and guiding appropriate admissions. The consultant conducts community and professional outreach, presents program benefits, and coordinates closely with the clinical team to ensure smooth transitions of care. Ideal candidates are compassionate, strong communicators with healthcare sales, liaison, or hospice experience and a passion for mission-driven end-of-life care.
