Nurse Practitioner - PRN

Role Overview This position involves conducting comprehensive health assessments for members in their homes or facilities, focusing on medical history, medication adherence, social factors, and other health risks. The provider collaborates with physicians, case managers, and caregivers to improve care quality, identify undiagnosed conditions, and facilitate follow-up and referrals. Assessments can last up to one hour and include administering tests and documenting findings per protocols. Key Responsibilities Perform adult/geriatric assessments, including health education and referrals Administer point-of-care and lab tests as needed Coordinate with healthcare team members and family Support patient education and follow-up in members’ residences Qualifications & Skills Master’s Degree or comparable experience with NP licensure Current NP license with prescriptive authority in practice state Board certification in relevant adult or family health specialties Valid driver’s license and automobile insurance Strong communication, computer, and documentation skills CPR/BLS/ACLS certification; experience preferred but new grads encouraged Additional Information Flexible scheduling, including days, evenings, and weekends Competitive pay based on assessments completed, with potential earnings up to $11,500/month Support for licensure in neighboring states and career growth opportunities Inclusive culture emphasizing empathy, trust, diversity, and professional development #J-18808-Ljbffr Live Well Homecare
