RN Case Manager

Fairview Park Hospital is seeking a compassionate, safety-focused PRN RN Case Manager to support patient-centered care in Dublin, Georgia. You will assess patient needs, create care plans, coordinate services across clinical teams, and help patients transition safely between care settings. Work collaboratively with patients, families, physicians, nurses, and community providers to address barriers, connect people with resources, and promote better outcomes.
