Highlights Pay: $40.09 - $78.04 hourly Location: Newton, MA Facility: Mass General Brigham Home Care, Inc. Schedule: Full-Time, Day shift What You'll Do Provide high-impact, patient-centered care by completing thorough nursing assessments and creating individualized care plans. Deliver advanced, skilled nursing interventions in the patient’s home. Monitor patient status using remote physiologic monitoring and respond promptly to changes in condition. Coordinate comprehensive care, including diagnostics and medication reconciliation. Document care efficiently and accurately using Point-of-Care electronic records. Provide patient and caregiver teaching, support safe transitions, and assist with discharge planning. Manage your daily schedule with autonomy, aligned with patient needs. Requirements Education Graduate of an approved, accredited school of professional nursing required; BSN preferred. Experience Minimum 1 year of recent acute care, home care or emergency nursing experience required. Licenses & Certifications Current license to practice as a Registered Nurse in the Commonwealth of Massachusetts required. Current CPR certification required. #J-18808-Ljbffr
