Joliet Area Community Hospice Lightways Hospice and Serious Illness Care is seeking a Field Registered Nurse (RN) Case Manager to join our interdisciplinary team. This role focuses on skilled nursing assessment and hospice care to enhance comfort and health for patients and families in home and community settings.
