Family Medicine NP/PA

Community Care Physicians seeks a Family Medicine NP/PA for our Slingerlands practice. In this outpatient role, you’ll provide comprehensive primary care across the lifespan, including preventive visits, acute care, and chronic disease management. Work within a collaborative, physician-owned multispecialty group that prioritizes patient-centered care, quality, and work–life balance. You’ll be supported by experienced clinicians, care teams, and integrated services, with opportunities for growth across our Capital Region network and participation in quality improvement and community-focused initiatives.
