Hybrid Care Manager (RN/MSW) - Member Health Advocate
City of Lincoln
The City of Lincoln is seeking a full-time Care Manager who will serve as the sole communicator between members and the Plan. This position requires creating and reviewing person-centered service plans and providing ongoing health education to members. The ideal candidate should possess an RN or a Master's Degree in Social Work, with effective communication skills to enhance member experiences.
