Grane Hospice is seeking a passionate, dedicated Hospice On Call RN to join our team! Office Location: 115 Union Avenue Altoona, PA 16602

Coverage area: Travel throughout the territory is required for visits

Schedule: Friday 4:30PM-Sunday 4:30PM

You will make a difference in our patient's lives by providing safe patient care, in a emergency visit setting, deliver individualized nursing care plans to ensure our patients have an optimal care experience while working as part of an interdisciplinary team, supporting both patients and their families. Qualifications

• Graduate of an accredited nursing school with current licensure to practice in state of operation
• 1 year nursing experience required, 3+ years preferred
• Experience in hospice or a similar setting preferred
• Knowledge of the hospice philosophy of care
• Commitment to clinical and documentation excellence
