Key Responsibilities\n

  • Directly engage with clients experiencing major mental illnesses and severe emotional disorders, providing individualized therapy and support remotely.

  • Maintain accurate and timely documentation of client interactions, treatment plans, and progress notes in accordance with hospice and mental health standards.
  • \n
  • Collaborate with interdisciplinary teams, including physicians, nurses, and other healthcare professionals, to develop comprehensive care plans tailored to clients’ needs.

  • Participate actively in team meetings, contributing insights and updates that enhance the collective understanding of client caseloads.

  • Implement crisis intervention strategies for at-risk clients, assessing their needs and coordinating appropriate resources.

  • Educate clients and their families about coping strategies and available resources related to mental health and end-of-life processes.

Patient Population / Client Focus\n

In this role, you will predominantly work with individuals facing terminal illness, adjusting to end-of-life care, and managing emotional distress. These clients will often also deal with complicated family dynamics and existential concerns, necessitating a nuanced approach to therapy. Your expertise will help them navigate these challenges, fostering environments where choices and empowerment are emphasized.

  • A Master's degree in Social Work from an accredited institution is required.

  • The candidate must hold a valid clinical social worker license (LCSW) in California.

  • We offer a competitive salary ranging from $70,000 to $85,000 annually, commensurate with experience.
