Overview Jackson Therapy Partners seeks a patient-centered Physical Therapist to provide high-quality care across diverse clinical settings. In this role, you will perform comprehensive evaluations, design evidence-based treatment plans, deliver therapeutic interventions, and document progress using EMR systems. You will collaborate closely with multidisciplinary teams while educating patients and families on home programs and long-term wellness. Responsibilities Perform comprehensive patient evaluations and document findings. Design and implement evidence-based treatment plans. Deliver therapeutic interventions to support recovery and mobility. Document progress in EMR systems and maintain accurate records. Collaborate with multidisciplinary teams to coordinate care.
